Telehealth Informed Consent

This Telehealth Informed Consent ("Consent") describes the telehealth services offered by Overtime Men's Health ("Overtime," "we," "us") and the benefits, risks, and limitations of receiving care through them. Please read it carefully. By checking the consent box, creating an account, or using the Services, you acknowledge that you have read, understood, and agree to this Consent.

What Telehealth Is

Telehealth is the delivery of health-related services and information through secure electronic and telecommunications technologies. It may include the electronic transmission of your health information, secure messaging with your care team, review of laboratory results, and, where clinically appropriate, consultations with and prescriptions from licensed providers. Your care may be delivered synchronously (live) or asynchronously (store-and-forward), depending on your needs and applicable law.

The Services We Provide

Overtime offers a men's health membership that may include diagnostic bloodwork, review of your results, personalized protocols, and, where appropriate and lawful, provider consultations and prescriptions fulfilled through partner pharmacies. Membership itself is not medical care and does not guarantee that any particular test, medication, or treatment will be recommended, ordered, or prescribed. All clinical decisions are made by licensed providers based on your individual health information.

Provider-Patient Relationship and Licensure

Clinical services are provided by licensed healthcare providers who are authorized to practice in the state where you are located at the time of the encounter. A provider may determine, in their sole professional judgment, that telehealth is not appropriate for your condition and that you should be seen in person or seek other care. A provider may decline to prescribe any medication or order any test if it is not clinically appropriate.

Benefits

Potential benefits of telehealth include improved access to care, convenience, more efficient evaluation and monitoring, and the ability to review your health data over time. Telehealth may reduce the need for travel and in-person visits for routine care.

Risks and Limitations

  • Information transmitted may not be sufficient to allow for appropriate medical decision-making, and the provider may request additional evaluation or in-person care.
  • Delays in evaluation or treatment could occur due to technology failures, connectivity issues, or the asynchronous nature of some services.
  • In rare cases, security protocols could fail, causing a breach of privacy of personal health information; we use administrative, physical, and technical safeguards to reduce this risk.
  • Telehealth is not a substitute for emergency care, and there are conditions that cannot be adequately assessed or treated remotely.

Not For Emergencies

The Services are not for medical emergencies. If you are experiencing a medical emergency, call 911 or go to the nearest emergency room immediately. Do not use secure messaging or any part of the Services to communicate emergencies or time-sensitive medical concerns, as responses may be delayed.

Prescriptions

Any prescription is issued solely at the discretion of a licensed provider and only when clinically appropriate. We do not guarantee that you will receive a prescription. Certain medications, including controlled substances, may not be prescribed via telehealth or may be subject to additional requirements under state and federal law.

Your Responsibilities

You agree to provide accurate, complete, and current information about your health, identity, medications, and location, and to update it as it changes. Providing inaccurate information may lead to inappropriate care recommendations. You are responsible for following the care instructions you receive and for seeking in-person or emergency care when advised or when your condition warrants it.

Privacy and Medical Records

Your personal health information is handled in accordance with our Privacy Policy and applicable law, including HIPAA where it applies. Clinical records generated through the Services are maintained in the electronic health record of the provider network delivering your care. You may request access to your records as described in our Privacy Policy.

Communications

With your consent, we may contact you by email and text message about your account, appointments, results, and care. Message frequency varies and message and data rates may apply. You can opt out of text messages at any time by replying STOP, and reply HELP for help. SMS consent is not a condition of receiving care. Voluntary Consent and Right to Withdraw

Your participation in telehealth is voluntary. You may withhold or withdraw this Consent at any time, without affecting your right to future care or treatment, by contacting us using the details below. Withdrawing consent will end the delivery of telehealth services to you going forward.

Acknowledgement

By checking the consent box or using the Services, you acknowledge that you have read and understood this Telehealth Informed Consent, that your questions have been answered to your satisfaction, and that you consent to receive care via telehealth under these terms.
